IMMUIZATION & SCHOOL HEALTH REQUIREMENTS

Florida state law requires that all students attending Florida schools have certain immunizations and a current school health exam. (Public/non-public Preschools and schools: section 1003.22, Florida Statutes, and Rule 64D-3.046, Florida Administrative Code) Failure to comply with these requirements could result in suspension from St. Paul Christian School.

 

The Supervisor of the Okaloosa County Health Department must sign a religious exemption of immunizations in order for your child to attend school if you choose not to immunize your child.

The following forms must be on file in the school office for your child to attend St. Paul Christian School:

  1. Current Florida Certificate of Immunization

  2. Current School Entry Health Exam

  3. Copy of Birth Certificate
